TeleGraft – Annual consortium meeting @ Porto

BMD Software was delighted to host this year’s Annual Autumn Consortium Meeting of the TeleGraft project in our home country, Portugal, in the beautiful city of Porto. TeleGraft is an European project focused on improving the quality of life for hemodialysis patients. BMD is proud to be a key partner in this initiative, contributing our expertise to enhance patient care and outcomes.

The two-day meeting brought together project partners to review the progress achieved, discuss ongoing research, and plan the next steps and challenges toward developing the smart arteriovenous graft – a significant progress in telemonitored home dialysis.

During the sessions, each team showcased their latest developments and results. BMD Software presented the technological platform that enables continuous remote monitoring of future patients. Currently we are developing a mobile and web solution designed to empower clinicians with decision support tools for patient management and monitoring. This platform will provide an automated alert mechanism that will warn of potential complications and infections by tracking patient biosignals. Additionally, it will enable the review of health surveys, access medical prescriptions, and maintain a detailed record of each patient’s progress.
